Insurance Requirements for Facilities Use

San Francisco State University requires that prior to the use of University facilities, all organizations that are not sponsored by a recognized University department to procure and maintain in full force during the term of the agreement, a policy of General Liability insurance evidencing the levels of coverage listed below. 

PLEASE NOTE:

  • The State of California, the Trustees, and the University, and the employees, officers and agents of each of them will not be responsible for any premiums or assessments on the insurance policy.
  • Failure on the part of the Lessee to maintain the required insurance in no way relieves the Lessee from the financial consequences associated with liability for the event.
  • The levels of coverage listed below are minimum levels of insurance.

TIMELINE

A certificate of insurance evidencing the required coverage should be requested at least 60 days prior to the event and must be provided to the Office of Conference and Event Services Risk Management at least 45 days (1.5 months) prior to the event. Failure to secure special event insurance 30 days (1 month) from your event may result in automatic postponement or cancellation of your event.

    The certificate must name San Francisco State University as an additional insured for claims involving bodily injury or property damage arising from the event. For any non-affiliated university groups or organizations, special event insurance can be purchased to meet this requirement.

    Commercial General Liability

    Commercial General Liability Insurance covering broad formcontractual liability, personal injury liability, advertising injury, completed operation and productliability, with a limit of not less than $1,000,000 per occurrence and $2,000,000 in the aggregate.General Liability coverage may increase depending on the services provided.

    Worker’s Compensation

    Worker’s Compensation Insurance with statutory benefits as required byCalifornia State Law.

    Business Automobile Liability

    Business Automobile Liability Insurance for Owned, Scheduled,Non-Owned, or Hired Automobiles with a combined single limits of not less than $1,000,000 peroccurrence.

    Employer Liability

    Employer Liability Insurance with a limit of not less than $1,000,000 peroccurrence.

    Excess/Umbrella Liability

    Excess Liability may be provided as supplement insurance to fulfillinsufficient coverages for Commercial General Liability, Worker’s Compensation, and BusinessAutomobile Liability.

    Abuse & Molestation

    (Only required if your event includes Minor participants.) Abuse &Molestation Insurance with limits of not less than $1,000,000 per occurrence and $2,000,0000aggregate.

    Additional Insured

    Due to the disclaimer which appears on the back or upper right hand corner ofmost certificates of insurance forms, San Francisco State University requires a separate additionalinsured endorsement form. The additional insured clause appearing in the description box of theface of the Certificate is not sufficient. THE CERTIFICATE OF INSURANCE MUSTSPECIFICALLY STATE THE VERBIAGE BELOW ON THE ADDITIONAL INSURED ENDORSEMENT FORM FOR THE INSURANCE DOCUMENTATION TO BE VALID.

    • All insurance required under this agreement is to be provided on a primary and non-contributing basis and must be reflected as such in the required insurance certificate. 
    • Insurance shall have an A.M. Best’s rating of at least A: VII.
    • The Certificate of Insurance shall include a description of the event, the date of the event, and the Lessee’s name or name of Lessee’s organization.
    • The Certificate of Insurance shall provide thirty (30) days advance written notice to San Francisco State University of any insurance policy cancellation.
    • Lessee’s insurance policy must be in effect at all times during scheduled activity, including early arrivals and late departures.
    • A copy of the certificate of insurance with the minimum insurance requirements shall be submitted to San Francisco State University a minimum of 10 working days prior to the commencement of Lessee’s scheduled event of said leased University property and submitted to Certificate Holder.
